Donor Tax REceipt Information and Forms
Thank you again for choosing to generously support the Children's Foundation, Adopt-A-Family Program to bring hope and joy home for the holidays!
Upon request, we can issue tax receipts for donations of $20 and up.
In order to issue a charitable receipt for income purposes, purchase receipts must be attached to a completed Tax Receipt Form. This includes gift, toy, and grocery/gift card receipts (not activation slips) for purchases donated to the Adopt-A-Family Program (AAF).
We can only issue a tax receipt for the amount of purchase receipts included. If purchase receipts include purchases for items other than those being donated to AAF, please remove them from the amount and cross them out before submitting the receipt.
If you have already dropped off your gifts and did not provide the purchase receipts and completed tax form, but would like a Charitable Tax Receipt for your donation, you can complete the form, take a photo of your receipts and email it to our Finance and Operations Director, Laurie, at email@example.com.
